KATHMANDU, April 18: Madhesh Police have formed a three-member investigation committee to probe the lapses in the police investigation in the death of Anandi Devi Sada from Donwari Tole, Nawarajpur Rural Municipality-3 of Siraha, who was found dead after allegedly being sexually assaulted.
The Madhesh Province Police Office (PPO) has formed the committee under the leadership of Senior Superintendent of Police (SSP) Shanti Raj Koirala. SSP Koirala informed Republica that the committee also includes Inspector Bikram Sah and one constable from the same office.
According to SSP Koirala, the investigation committee was formed on Wednesday, and the probe has already begun.
Chief of the PPO Deputy Inspector General Lal Mani Acharya said that the committee has been tasked with completing the investigation as soon as possible and submitting a report once the medical reports are received.

SSP Koirala revealed to Republica that the team will not just be investigating the cause of death of Sada but also probe into the reported lapses in the police investigation into the death.
“We have been tasked to determine the cause of death but also the alleged mishandling of the case by the local Area Police Office,” SSP Koirala said, “We need to determine whether the death was caused by mental pressure exerted by the perpetrators, societal pressure, or due to the police’s lapses.”
Aanandi Devi, who was sexually assaulted on December 24, 2025 by two men from her own village, was found dead at her maternal home on Tuesday afternoon. The accused has been identified as Sanjeev Kumar Yadav, 23, and Shyam Sundar Yadav, 32.
Although the victim filed a complaint at the Area Police Office in Maheshpur Patar on the day of the incident, she could not receive justice due to police delays.
“All the media reports about the delays in investigation have been noted by our team,” SSP Koirala said, “We will investigate the effect of the alleged delays in registration of the case.”
Although the police registered a complaint on February 10 and initiated an investigation, the youths involved in the incident are still absconding. The family suspects that the victim may have committed suicide due to the stress of not receiving justice and her husband's neglect after the incident.
SSP Koirala added that the similar case of rape and death of Rinku Sada who was abducted and gang-raped by three men from Nawarajpur Rural Municipality of Siraha district on February 9 will also be taken into account during the investigation.
“We will also investigate whether the pressure for reconciliation on the victim by the perpetrators and the society, like in the case of Rinku, had any effect on the death,” SSP Koirala said.
